ATTENTION: Important Clinic Information

Due to requirements related to COVID-19, we are screening and re-screening all patients.  Patients with negative screenings may be scheduled for an in-clinic visit.  Other patients will be scheduled for Telehealth visits.  We will continue to offer Telehealth visits, curbside medication pick-ups and allergy injections by appointment only.  
Call 615-346-9437 if you have any questions or to be screened and scheduled for an in-clinic or Telehealth appointment.  If we are unable to answer, please stay on the line and leave a detailed message so we can return your call.  Messages are monitored 24/7.  Please continue to monitor this site regularly for updates.

Patient Portal Information and Instructions 

Online appointment scheduling is now available! Click the "How to Schedule an Appointment Online Using Your Patient Portal" button below to view instructions on how to schedule an appointment using your online patient portal. DOT physical appointments cannot be scheduled online. Please call 615-346-9437 to schedule your DOT physical appointment.  In addition to scheduling appointments, your patient portal provides information about your allergies, medications, prescriptions, lab results and more in one convenient location.  To register for your patient portal, click the "REGISTER FOR YOUR PATIENT PORTAL" button below and click the "HOW TO REGISTER" button to see step by step instructions on how to register for your patient portal. 

If you do not wish to register for your patient portal at this time, your health management team will send you an invitation via email to register for your portal.  You will receive an email with the subject line "Welcome to your Patient Health Record".  The email will then guide you on how to register for your patient portal.  Once registered you may access your patient portal anytime.  Click the "PATIENT PORTAL" button below to sign in. Instructions on how to register via the email invitation are also provided below.
