Online appointment scheduling is now available! In addition to scheduling appointments, your patient portal provides information about your allergies, medications, prescriptions, lab results and more in one convenient location. To register for your patient portal, click the "REGISTER FOR YOUR PATIENT PORTAL" button below and click the "HOW TO REGISTER" button to see step by step instructions on how to register for your patient portal.
If you do not wish to register for your patient portal at this time, your health management team will send you an invitation via email to register for your portal. You will receive an email with the subject line "Welcome to your Patient Health Record". The email will then guide you on how to register for your patient portal. Once registered you may access your patient portal anytime. Click the "PATIENT PORTAL" button below to sign in. Instructions on how to register via the email invitation are also provided below.
