What is Elation Passport? Elation Passport allows your provider to send messages to your Passport like an email. You get notified by email or text to check your Passport for the new message from your provider. From Passport, you can read your provider's letter and view any attached documents- like lab results.
You may also access your health information, such as allergies, current medications, etc. Notes from each visit will automatically be available to you online.

Account Set Up: Contact the City of Clarksville Employee Health and Wellness care team to set up your account. 
They will send you an invitation to activate your account. Patients are required to provide an active email account where the invitation will be sent, and patients must provide a cell phone number where an activation code will be sent. 
If you are unable to provide an email or cell phone number, please speak with your care team to discuss an alternative option to set up your Elation Passport account. 

Activate Your Account
  1. You will receive an invitation email from "[email protected]" with the subject line [Provider's Name] has a secure message for you.
  2. Open the email and click the "Click here to Register for Passport" button. 
  3. A new page will open and prompt you to enter a 7-digit Passport Invitation Code and to create a password. The 7-digit Passport Invitation Code will be texted to the cell phone number stored in your chart. 
  4. You must enter the 7-digit Passport Invitation Code, create a password, and enter it twice. Agree to Elation's Terms of Service and specify if you are a patient or a legal representative of the patient before you can complete registration.
  5. Once all fields are complete, click "Submit" to complete registration, and then you will be brought into your Passport account.
